We are committed to supporting parents and carers as learners prepare for life beyond school.
We start planning for adulthood as early as Year 11 (ages 15–16) to make sure the move is smooth and well-informed.
- Working Together: We liaise with the Adult Transition Team, who provide guidance and support during your child’s Annual Reviews.
- Exploring Options: Parents and carers are invited to transition events with local providers, giving you the chance to learn about future opportunities and services.

- Additional Support: You can also reach out to your Social Care Team or your Local Council’s Transition Team for extra resources and advice.
