We start planning for adulthood as early as Year 11 (ages 15–16) to make sure the move is smooth and well-informed.

• Working Together: We liaise with the Adult Transition Team, who provide guidance and support during your child’s Annual Reviews.
• Exploring Options: Parents and carers are invited to transition events with local providers, giving you the chance to learn about future opportunities and services.
• Need Help or Have Questions? If you have any concerns about your child’s transition at 18, please contact our Careers lead, Anna- careers@wrenspinney.co.uk
• Additional Support: You can also reach out to your Social Care Team or your Local Council’s Transition Team for extra resources and advice.
